6. Original Affidavit will be sent to you to file with the court, along with a file copy.

2. Fill out the one-page "Notice for Publications".
(sent with your approved articles - if not, email us for the form). Notice must be filled out completely to receive minimum charge.
3. Fax or email to us with a cover page with the following information:

4. We will send you an invoice.

6. We will send Original Affidavit to the Arizona Corporation Commission.
7. You will receive copy of Affidavit.
